Learning objectives
Track 3 focuses on the integrated management of chronic conditions, emphasizing a structured and patient-centered approach. The track covers the methodology and design of care pathways for the prevention, early diagnosis, and effective management of chronic diseases. Students will learn how to tailor interventions to the individual patient's disease history, optimizing the efficacy of treatments and promoting adherence to therapeutic protocols. The course highlights the importance of coordinated care, multidisciplinary collaboration, and continuous monitoring to improve outcomes and quality of life for patients with chronic conditions.
Expected learning outcomes
Upon completion of the track, students are expected to understand the principles and methodology for the prevention, diagnosis, and management of chronic conditions.
